Please complete the form as honestly and accurately as possible. There are no right or wrong answers. The information you provide will help create a clearer understanding of your child or young person's strengths, challenges, behaviours, mindset, and development needs.
Please note:
• This form is intended for parents and guardians only.
• This form must not be completed by the young person being referred.
• This form is not intended for schools, colleges, alternative provisions, sports academies, football programmes, or any other professional organisation.
• A separate Young Person Information Form will need to be completed before the Snapshot review can begin.
