Parent & Guardian Referral

If you're here, you're likely looking for a clearer understanding of the challenges your child or teenager is currently facing and what practical steps may help them move forward.
Power Path Snapshot provides an independent mentoring-based review designed to identify strengths, highlight potential barriers, and offer clear guidance for development. It is suitable for young people who may be experiencing difficulties with confidence, motivation, behaviour, mindset, engagement, personal responsibility, emotional regulation, or general direction.
Using information gathered from both parent and young person perspectives, Power Path Snapshot creates a clearer picture of what is going well, where support may be needed, and what positive next steps could be considered.
The aim is not to label, diagnose, or judge. The aim is to provide clarity, encouragement, and practical guidance that helps young people recognise their strengths and better understand the areas that may be holding them back.
What's Included
Following completion of all required forms, you will receive:
• A structured PowerPath Snapshot review

-
Read the information on this page to ensure Power Path Snapshot is the right fit for your child or young person.
-
Click the Parent & Guardian Referral Form button below.
-
Complete the Parent & Guardian Referral Form and submit the service fee payment. Your referral can only be submitted once all required information and payment have been completed.
-
Once your referral has been successfully submitted, you will receive a confirmation email containing a unique link to the Young Person Information Form.
-
Share this link with your child or young person and ask them to complete the form independently and honestly.
-
Once the Young Person Information Form has been submitted, you will receive a second confirmation email confirming that the Snapshot review process has begun.
-
I will review all submitted information using the Power Path Snapshot framework to identify strengths, development opportunities, behavioural patterns, and key areas for focus.
-
Within 48 business hours of all required forms being completed, you will receive:
• A written Power Path Snapshot review
• Practical recommendations and suggested next steps
• A personalised video breakdown explaining my findings and guidance -
The Snapshot review concludes the service unless additional mentoring support is requested separately.
Participation & Referral Requirements
Power Path Snapshot is designed to be a collaborative process between parent or guardian and young person.
Before submitting a referral, please ensure that your child or young person:
• Is aware that a referral is being made
• Understands what PowerPath Snapshot is
• Has agreed to complete their part of the process
The Snapshot review cannot begin until all required forms have been completed.
As part of the referral process, parents and guardians will be asked to confirm that the young person is aware of and has agreed to participate.
Please note that referral fees are non-refundable once a referral has been submitted and processed. This includes situations where the young person later chooses not to complete their required form or engage with the process.
For this reason, we strongly recommend discussing the referral with your child or young person before proceeding.
